That's one of the reasons that we all say we seek disconfirming evidence, but in many contexts we don't actually.

To get accountability requires disconfirming evidence.

To have disconfirming evidence requires an other to have the ability to audit and raise issues.

The other has to not feel pain directly when you do.

This is one of the reasons things like an independent review board are sometimes necessary.
